  12. 19________. I, Joseph Tourmolin, being of sound mind, willfully
  13. and  voluntarily make known my desires that my dying shall not be 
  14. artificially prolonged under the circumstances set forth below, 
  15. do declare:

  17.     If at any time I should have an incurable injury, disease, or
  18. illness certified to be a terminal condition by two physicians who
  19. have personally examined me, one of whom is my attending
  20. physician, and the physicians have determined that my death will
  21. occur whether or not life- sustaining procedures are utilized and
  22. where the application of life-sustaining procedures would serve
  23. only to artificially prolong the dying process, I direct that such
  24. procedures be withheld or withdrawn, and that I be permitted to
  25. die naturally with only the administration of nutrition,
  26. medication or the performance of any medical procedure deemed
  27. necessary to provide me with comfort, care or to alleviate pain.

  29.     In the absence of my ability to give directions regarding the use
  30. of such life-sustaining procedures, it is my intention that this
  31. declaration shall be honored by my family and physicians as the
  32. final expression of my legal right to refuse medical or surgical
  33. treatment and accept the consequences from such refusal.

  35.     I understand the full import of this declaration and I am
  36. emotionally and mentally competent to make this  declaration.
